Temperatures reached 51° C (123°F) and killed 3,000 people in India in 1998 as a whole. In June, 1994 temperatures soared to 49.5 ° C (121° F) in Rajasthan, leaving more than 400 people dead from heat related illnesses. One six-year old girl died of a heat stroke walking from the village bus stand to her rural hut. A construction worker told the Washington Post, "It is especially difficult for laborers. Sometimes everything goes hazy before my eyes. I get dizzy. I constantly have to keep watch on myself." At the same time, New Delhi thought it was going through a cold spell when temperatures dropped to 37° C (98°F) . Coca Cola got into trouble when it ran an ad campaign during hot spell which read "Thank God for the heat wave."

The India government said the heat had killed 3,500 people from 2015 to 2019. A heat wave in early summer of 2003 left more than 1,300 people dead, many from sunstroke and dehydration. Among the areas hardest hit were the southern state of Andhra Pradesh, where temperatures reached 49°C (120°F) and more than 1,200 people died. Temperatures reached 43° C (109° F) in Calcutta. The heat wave finally dissipated when the monsoon rain arrived, In 2002, a similar heat wave killed more than 1,000.

As global and local temperatures have risen, heatwaves have become regular occurrances in India — particularly in May and June before the onset of the monsoons. Describing a heat wave in May 2020, AFP reported: India is wilting under a heatwave, with the temperature in places reaching 50°C (122°F) and the capital enduring its hottest May day in nearly two decades. The hot spell is projected to scorch northern India for several more days, the Meteorological Department said, "with severe heat wave conditions in isolated pockets". Met officials said Churu in the northern state of Rajasthan was the hottest place on record at 50°Cs, while parts of Punjab, Haryana and Uttar Pradesh sweltered in the high 40sC (110sF).. The country of 1.3 billion people suffers from severe water shortages with tens of millions lacking running water — to say nothing of air conditioning. Parts of Delhi and elsewhere regularly see scuffles when tankers arrive to deliver water. In 2019, Chennai made international headlines when the southern city ran out of water entirely. During the 2020 heatwave, the BBC reported: The Indian capital, Delhi, saw temperatures rise to 47.6° C (117.7°F). The severe heatwave conditions occurred as thousands of migrants are walking on the highways after fleeing cities to try and go back to their villages. Many are walking with little food and water, and will be among the most exposed to the weather. In addition to that, the heatwave has impacted efforts to combat swarms of locusts that are destroying crops in parts of the country, including Rajasthan. Huge swarms of desert locusts, have destroyed nearly 50,000 hectares (125,000 acres) of crops across western and central India, and may enter Delhi in coming days. More than 100 workers are battling the insects, using vehicle-mounted sprayers, pesticides and drones in the searing heat.

But the heat wave [in 2022], was unprecedented in its severity, duration, and geographic expanse. Across much of northern India, where more than a billion people live, temperatures have regularly soared past 43.3°C (110°F)and slightly lower temperatures have often combined with very high humidity—a dangerous combination. “The heat is rising rapidly and much earlier than usual,” Prime Minister Narendra Modi said, in April 2022. “Fire has broken out in many forests, historical monuments, and hospitals.” Indians who work outside—about half the population—have sometimes had to stop in the afternoons, relinquishing their wages; schools and businesses have had to adjust their hours or shut entirely; and farmers have seen their crop yields drop by a third or more. Only eight per cent of Indians have air-conditioning, and many lack reliable electricity, a situation that limits their use of fans and other cooling devices...”It didn’t use to be this hot,” Saira, the woman in a poor area of Delhi said, “Before, it felt like it was possible for humans to work the landfill.” Now, because of the heat, they tried to stay out of the sun. “If you see five hundred people working there right now, you’ll see at least two thousand people up there at night,” she said. We eat up there, sleep up there sometimes,” another worker added.

Hema, a thin woman in a purple sari, sat on the stairs. “When the sun hits, it feels like your body is on fire,” she said. “I drape a shirt over my head—that makes it feel even hotter. When we come back home, our heads feel like they will explode. We take water with us, but it’s boiling by the time we can drink it.” The women described headaches, exhaustion, dizziness, rashes, fever. The stench of the landfill—an acrid mixture of excrement and rotting trash—was sickening, they said, but the heat made it hard to tolerate masks. Outside the hut, children kicked a ragged soccer ball. A scrawny dog panted on a mound of refuse. Flies swarmed a heap of dung. We are living,” Saira said. “But we are also dying.”

The Paharganj Mohalla Clinic, in central Delhi, is one of more than a hundred public clinics in the city. Deepika Sharma, the clinic’s doctor, stood in the second room, The clinic was busy, Sharma told me, but not unusually so. She typically sees a hundred patients a day, with each visit lasting about three minutes. As a physician myself—I practice internal medicine at a large academic hospital in New York—I found this pace dizzying to contemplate. On a normal day, I might see fifteen or twenty patients. But Sharma’s task was made easier, she said, by the fact that so many patients had the same problems. Nearly half the people she saw had respiratory issues—such as asthma, bronchitis, and emphysema—which she attributed to the city’s punishing air pollution. (India has among the world’s highest rates of chronic respiratory disease; on the worst days, breathing the Delhi air is equivalent to smoking two packs of cigarettes.) During the heat wave, she said, around a quarter of her patients suffered heat-related rashes, and a fifth presented with signs of dehydration. Inhalers, calamine lotion, oral rehydration salts—these three medications made up the bulk of her prescriptions.

Public clinics refer their sickest patients to public hospitals. One such institution, Civil Hospital, is situated just off a highway in Palwal, a city of a 350,000 people outside of Delhi. Brahmdeep Sindhu, its chief medical officer, said the heat has created a rise in physical, psychological, and social symptoms,” he said. “Heatstroke, dizziness, low blood pressure, dehydration, exhaustion—we are seeing these almost daily.” The hospital was on a main road, and often admitted travellers. “The A.C.s in cars can’t function at such high temperatures,” he said. “The cars become extremely hot. The other day, three passengers came in—one person vomiting, one person with fever, one person so weak he couldn’t move.”

A psychiatrist by training, Sindhu was especially troubled by a surge in psychological distress caused by the heat. “Patients with bipolar disorder, with schizophrenia—they are really struggling,” he said. The Intergovernmental Panel on Climate Change, in its 2022 report, highlighted for the first time the dire mental-health effects of a warming planet: anxiety, grief, stress, post-traumatic stress disorder. In recent years, heat has been linked to a rise in suicides among Indian farmers, whose livelihoods have been imperilled.

High Temperatures Killing More Indians

A study published in The Lancet in October 2022 found that deaths linked to extreme heat in India increased by 55 percent between the periods 2000–2004 and 2017–2021. The study also estimated that exposure to high temperatures resulted in the loss of 167.2 billion potential labor hours in 2021. According to the report, these lost labor hours were associated with income losses equivalent to approximately 5.4 percent of India's gross domestic product (GDP). Although heatwaves have long occurred in many parts of the country during the summer, researchers and climate experts report that they are becoming more frequent, longer-lasting, and more intense. The annual “Lancet Countdown” report, which examined conditions in 103 countries, found that the March–April heatwave affecting India and Pakistan was about 30 times more likely to occur because of climate change. A separate study by the United Kingdom's Met Office similarly concluded that climate change had made record-breaking heatwaves in northwestern India and Pakistan about 100 times more likely. Without climate change, such extreme temperatures would be expected to occur only once every 312 years. The report noted that extreme heat can directly affect human health by increasing the risk of heat stroke and worsening existing cardiovascular and respiratory conditions. It can also contribute to adverse pregnancy outcomes, sleep disruption, mental health challenges, and higher rates of injury-related deaths, with vulnerable populations facing the greatest risks.

True death tolls are likely much higher than official counts. Dhruv Khullar wrote in the The New Yorker: In 2010, during a heat wave in Ahmedabad, the financial center of the state of Gujarat, officials counted 75 heatstroke deaths during the hottest week—but a later analysis of death certificates revealed that there had been at least 800 more deaths than usual during that time, some 200 of them on a single day. Research has shown that, each day the temperature rises above ninety-five degrees in India, the annual mortality rate increases by three-quarters of a per cent. In 2015, one of India’s deadliest heat waves in recent history caused more than 1,100 deaths and affected large parts of the country. Temperatures remained exceptionally high for several weeks, with New Delhi recording highs of about 45°C (113°F). In parts of eastern and central India, temperatures climbed even higher, reaching nearly 48°C (118°F), while some locations reported temperatures above 50°C (122°F). In the past, similar fires had been extinguished within hours or days, but Bhalswa burned for weeks. “The weather poses a big challenge for us,” Atul Garg, the chief of the Delhi Fire Service, said, nine days after the fire began. “Firefighters find it difficult to wear masks and protective gear because of the heat.” A nearby school, blanketed by hazardous smoke, was forced to close. In the end, it took two weeks to extinguish the blaze. The charred bodies of cows and dogs were found in the debris. When I visited, in May, some sections of the landfill were still releasing angry coils of smoke. In the car, I consulted my phone, which told me that it was a hundred and three degrees outside, with thirty-two-per-cent humidity.
